One of our clients is seeking Truancy Case Managers & Family empowerment Services Case Managers (Bi-Lingual wanted but not required) in the Philadelphia, PA area. (up to $4,000/month)

SUMMARY OF CLIENT'S DESCRIPTION OF THIS OPPORTUNITY
We are seeking dedicated Case Managers to support individuals and families, managing a caseload of up to 35 clients. The Case Manager will provide direct services through home visits, phone calls, office meetings, and written communication. This role includes conducting outreach to raise awareness of agency programs, visiting schools to meet with staff and students, and collecting necessary documents. The Case Manager will also make appropriate referrals, accompany clients to appointments when necessary, and represent them in Regional Truancy Court and Family Court.

Coordination with other agencies to connect clients with additional services is essential. The position requires maintaining accurate and up-to-date case files, including data entry and detailed case notes.

CLIENT'S REQUIRED SKILLS & EXPERIENCE

* Bachelor's Degree in Social Work or related field

* 2 year experience in human services preferably child care

* Ability to complete and pass clearances: FBI, Child Abuse, PA State Background Check, & Act 31.

* Must have valid driver's license & provide Motor Vehicle Records

* Provide documentation of: CPR Certification, First Aide Level 2 Certification.

* Must be able to produce current medical/health documents including:
Physical examination & Tuberculosis (TB) test
