Description:
Program Specialist needed for full-time, temporary (2-3 months) position in Philadelphia County. The Program Specialist coordinates referrals, intake, assessments, enrollment, and service planning for individuals with intellectual and developmental disabilities. Responsibilities include monitoring treatment interventions and outcomes, coordinating services and activities, maintaining documentation, reviewing records for compliance, and serving as a liaison with families, service coordinators, and community resources. The role also provides staff training, supervision, and support related to service plan implementation while ensuring quality services and adherence to agency standards.
Position Requirements: BSW with at least 2 years of direct experience supporting individuals with I/DD, or MSW with 1 year of direct experience. Knowledge of HCSIS, HRST, and EHR systems preferred.
