Description:
Aging Service Coordinator - FT
- Temp to Hire
- Philadelphia County
Conducts assessments, develops service plans, coordinates and monitors care, supports transitions, maintains CHC eligibility, and ensures accurate documentation and HIPAA compliance. Participate in quality improvement and team activities.
Position Requirements:
Requires BSW, three years of relevant experience, a valid driver's license, a good driving record, and access to an insured vehicle.
