Case Manager needed for full-time, temporary (4 months) position in Philadelphia. Join a grant-funded community outreach program serving individuals experiencing housing instability and other barriers to care. Responsibilities include outreach, case management, care coordination, connecting clients to medical and community resources, and maintaining program documentation.
BSW and case management experience required.
