LTC Social Worker needed for part-time, temporary position in Delaware County. We are seeking a Social Worker to provide psychosocial support and services to residents in a continuing care retirement community under the supervision of the Director of Social Services. Responsibilities include assisting with discharge planning, completing MDS documentation, addressing residents' psychosocial needs, and collaborating with the interdisciplinary team.
BSW is required. Experience with family systems, the aging population, discharge planning, and prior work in a CCRC or long-term care setting is preferred.
