DESCRIPTION OF WORKThis position focuses on leading transitional services that help youth successfully return to their communities. It involves planning, coordination, and guidance that support safety, growth, and long‑term stability. As a Social Worker 2, you will perform the following duties:
- Transitional Planning:
Develop plans that support youth as they prepare to return to their communities - Safety Oversight:
Maintain a secure environment through supervision and preventive action - Case Coordination:
Work with families, probation officers, and facility staff to support aftercare goals - Resource Networking:
Build connections with community programs that help youth access important services - Documentation Duties:
Record assessments, updates, and reports that outline resident progress - Counseling Support:
Provide individual and group guidance to help residents
