Registered Dietitian; RD

Complete Care Management is seeking a Registered Dietitian (RD) to support two skilled nursing facilities
: Complete Care at Phillipsburg and Complete Care at Brakeley Park
. This role is ideal for a dietitian who enjoys variety, collaboration with interdisciplinary teams, and making a direct impact on resident health and outcomes.

• Assess residents’ nutritional needs and develop individualized care plans
• Conduct nutrition assessments and participate in care plan meetings
• Monitor weight changes, labs, and dietary compliance
• Collaborate with nursing, dietary, and clinical teams to ensure quality nutrition services
• Provide nutrition education to staff and residents as needed
• Ensure compliance with state, federal, and CMS regulations
• Support survey readiness and address nutrition-related deficiencies
• Review menus and therapeutic diets to meet resident needs

• Registered Dietitian credential (RD/RDN) required
• Experience in skilled nursing or long-term care preferred
• Strong knowledge of CMS regulations and long-term care nutrition standards
• Excellent organizational and communication skills
• Ability to manage time effectively while covering multiple locations
