This is a full-time on-site role for a Home Care Physical Therapist based in Phillipsburg, NJ. The Home Care Physical Therapist will be responsible for assessing and developing personalized rehabilitation plans for patients in their homes. Day-to-day tasks involve performing physical therapy interventions, assisting with rehabilitation exercises, monitoring patient progress, and collaborating with patients, families, and healthcare teams to promote optimal well-being.

The therapist will also educate patients and their families on exercises and strategies to improve mobility and overall health.

• Proficiency in Physical Therapy and Rehabilitation techniques
• Strong knowledge of Medicine and Patient Care
• Experience providing Home Care services
• Exceptional interpersonal and communication skills to work effectively with patients, families, and other healthcare professionals
• Valid Physical Therapy license in the state of New Jersey
• CPR certification and knowledge of safety protocols
• Reliable transportation to patient locations
• Prior experience in home health or similar care settings is a plus
