Seeking a compassionate and skilled LPN to provide high‑quality, individualized care to residents during the evening shift. The role involves administering medications, collaborating with interdisciplinary teams, and supporting residents and families.

• Provide direct patient care to a diverse population of seniors, assessing, planning, and implementing individualized care plans.
• Administer medications, treatments, and therapies as prescribed by physicians and other healthcare professionals.
• Collaborate with interdisciplinary teams to ensure comprehensive and coordinated care.
• Monitor and report patient status to nursing staff, physicians, and other healthcare professionals.
