BACKGROUND
The Federal Bureau of Prisons is awarding an indefinite delivery, requirements type blanket purchase agreement to a responsible entity for the provision of a Dental Hygienist. The Dental Hygienist will provide services to the inmate population at the Federal Correctional Institution/Federal Prison Camp (FCI/FPC) Phoenix, Arizona.
PLACE OF PERFORMANCEDental Hygienist services will be provided in the FCI/FPC Phoenix Health Services Department located at 37900 N. 45th Ave, Phoenix, AZ 85086. The duties of this position require frequent and direct contact with individuals in confinement who are suspected or convicted of offenses against the laws of the United States.
SCHEDULE OF PERFORMANCEWork hours may vary depending on the need of the institution. Shifts typically run from 6:00 AM to 3:00 PM, Monday through Friday, exclusive of federal holidays. A shift equals one (1) hour and can include up to 8 sessions per day with a thirty‑minute unpaid lunch period if applicable. The schedule shall not exceed forty (40) sessions per week, as defined by the Health Services Administrator (HSA) or designee.
Schedules may be adjusted based on institutional needs; any changes must be coordinated with the HSA. Delays due to weather, mechanical issues, or security matters may affect service delivery. In such cases, the contractor will be paid for two (2) sessions. Remaining sessions for the day may be suspended, and the contractor may be released pending institution request.
The institution will provide all supplies and equipment required by the contractor. All records maintained by the contractor remain the property of the institution. Contractors are expected to wear professional clothing; scrub uniforms are approved; denim, orange, and khaki are not allowed.
SCOPES OF SERVICE- Provide dental prophylaxis to inmates referred by medical unit staff.
- Provide thorough individualized oral hygiene instructions to each patient.
- Monitor oral hygiene and periodontal condition on each patient seen for prophylaxis.
- Assist the Chief Dentist and Staff Dentist when needed or necessary.
- Complete necessary dental x‑rays as specified by the dentist.
- Sterilize instruments for prophylaxis and other instruments as specified.
- Perform necessary paperwork related to dental healthcare and/or medical unit needs.
The contractor shall immediately inform the HSA and Administrative Contracting Officer in writing of any adverse action taken by a regulatory body involving certifications, including suspensions, revocations, or restrictions on licensure.
METHOD OF PERFORMANCEServices are professional in nature and will align with AAFCO standards of care, meeting local medical practice standards and adhering to all applicable federal, state, and local regulations. Services, though not directly supervised, will be reviewed periodically by the HSA and the Chief Dentist. The contractor retains professional responsibility for all patients referred to the Health Services Department.
EMPLOYEE HEALTH REQUIREMENTSThe contractor shall ensure that the Dental Hygienist is free of infectious diseases. Should the hygienist develop a medical condition that may pose risk to others, they will be immediately removed from invasive procedures and activities. The contractor will be responsible for securing any replacement personnel at no additional cost to the institution.
EDUCATION AND QUALIFICATIONSApplicants must be certified/ licensed as a Dental Hygienist in any state, submit photocopies of license and/or board certification, and possess dental radiology, anesthesia, and CPR certifications. A minimum of three years of experience is preferred.
