Position: Value-Based Primary Care Provider

Optima Medical is an Arizona-based medical group consisting of 30 locations and over 130+ medical providers, who care for more than 200,000 patients statewide. Our mission is to improve the quality of life throughout Arizona by helping communities "Live Better, Live Longer" through personalized healthcare, with a focus on preventing the nation’s top leading causes of death. We go beyond primary care with a full spectrum of services including cardiovascular health services, behavioral health, allergy testing and immunotherapy, in-house lab testing, imaging, chronic disease management, and other specialty health services.

Job Responsibilities:

  • Discuss and review patients' medical history, symptoms, allergies, and current medications
  • Perform comprehensive physical examinations and clinical assessments
  • Ask patients situation-specific questions to formulate accurate diagnoses
  • Diagnose and treat acute and chronic medical conditions
  • Develop and implement individualized treatment plans focused on improving patient outcomes
  • Prescribe suitable medications and provide proper dosage and administration instructions
  • Order, review, and interpret laboratory, imaging, and diagnostic test results
  • Provide preventive care services, wellness visits, health screenings, and patient education
  • Collaborate with care teams and specialists to coordinate comprehensive patient care
  • Manage patient panels and support value-based care initiatives, quality measures, and population health goals
  • Maintain accurate and timely documentation in eClinical

    Works (eCW)

Job Qualifications:

  • Doctor of Medicine (MD), Nurse Practitioner (NP), or Physician Assistant (PA)
  • Board-certified in Family Medicine, Internal Medicine, or a related field
  • Active Arizona license or ability to obtain prior to start date
  • Experience providing primary care services in an outpatient setting preferred
  • Sound medical knowledge
  • Experience utilizing electronic medical record systems, preferably eClinical

    Works (eCW)
  • Knowledge of value-based care models, quality measures, and population health management preferred
  • Excellent analytical and problem-solving skills
  • Exceptional communication skills
  • A patient and compassionate disposition
  • Detail-oriented
