Alignment Health is seeking a compassionate Health Coach & Educator to support members in Maricopa County, AZ. This field-based role conducts in-home visits, builds relationships with members and families, and provides wellness coaching and education to improve overall health and wellbeing.
Candidates should be comfortable driving locally, manage a daily schedule of visits, and travel within the service area. Mileage reimbursement is provided for travel from home to visits.
