As a Transplant Pharmacist
, you provide direct support for abdominal transplant services, including kidney, liver, and pancreas transplantation, across both inpatient and ambulatory settings. Pharmacists currently rotate through these responsibilities in two‑week intervals.

The position plays a critical role in collaborating with transplant providers, the Infectious Disease team, and the multidisciplinary transplant team. Pharmacists also participate in transplant selection committees, quality initiatives, process improvement efforts, and the development of policies and clinical protocols.

Schedule for this position will have daytime hours, Monday - Friday.

POSITION SUMMARY

As a pharmacotherapy expert, you actively participate on the health‑care team to provide quality patient care in the area of specialty. You participate in the optimal selection of drug therapy and design regimens to maximize therapeutic benefit and minimize toxicity. You are accountable for ongoing evaluation and documentation of all components of pharmaceutical care delivered to patients. You conduct educational activities, apply principles of performance improvement, and participate in Pharmacy and Therapeutics Committee/Clinical Consensus Group activities as appropriate.

You demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients you serve. You are accountable for departmental and individual patient outcomes by assessing and evaluating data reflective of patients’ status, interpreting the appropriate information needed to identify each patient’s requirements, and providing pharmaceutical care based on individual needs. You are responsible for the delivery of pharmaceutical care to patients using the medication use process.

You oversee the activities of pharmacy technicians, coordinating workflow and checking medications prior to dispensing. You adhere to local, state, and federal laws regarding the profession of pharmacy practice.

CORE FUNCTIONS
  • Critically evaluate disease‑state management and adjust or recommend adjustments to therapy to improve patient medication regimens. Ensure appropriate and cost‑effective medication therapy. Focus areas may include, but are not limited to, evaluating disease‑state management, medication management, and adverse drug reactions. Adhere to clinical practices approved by Pharmacy and Therapeutics Committee/Clinical Consensus Group (e.g., collaborative practice agreements). Accurately and completely document patient assessment data and clinical interventions.
