SpecialtyCase Manager
Job Summary
The Specialty Case Manager provides case management services in the community to individuals who have a mental health and/or substance use diagnosis, survivors of domestic violence and/or those identified as high risk due to their acuity level. The Specialist assists in planning, connecting, advocacy, coordination, and monitoring; to assist patients in gaining access to health services, financial assistance, housing, employment, education, social services and natural support.
They assess needs, develop and monitor individualized treatment plans and document services provided in an accurate and timely manner. The Specialty Case Manager embraces the recovery philosophy and promotes a safe recovery environment for clients. The Specialist will be responsible for implementing advanced techniques and interventions to serve the high‑risk individuals in their associated program.
- High school diploma or GED required. Associate’s degree or higher in a field related to behavioral health preferred.
- Recovery from alcohol or drugs and/or GMH for equal to or greater than one consecutive year preferred.
- 1–3 years of experience working with special populations (individuals experiencing serious mental illness, survivors of domestic violence, individuals determined high cost/high need preferred).
- 1–3 years of experience in a behavioral health position providing outreach and engagement activities preferred.
- Current Arizona Driver’s License (valid and in good standing).
- 39-month Motor Vehicle Record (MVR).
- Arizona Fingerprint Clearance card.
- Jail Clearance (if applicable).
