Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. CBI provides residential, outpatient, inpatient, medication-assisted treatment, and crisis services to individuals experiencing opioid use disorder, homelessness, and mental illness.
Lead NavigatorThe Lead Navigator is a professional in recovery from AOD use and/or GMH for at least one year and has a minimum of 90 days experience as a Navigator or Peer Support Specialist. The primary role is to work with homeless individuals, providing outreach and supportive services to help them end homelessness. The Lead Navigator coordinates and monitors RBHA and non-RBHA community-based services, facilitates delivery of services, and serves as a bridge between patients and community providers.
The Lead Navigator identifies individuals with the highest acuity and greatest vulnerabilities, conducts outreach, engagement, case management, and ongoing wrap‑around supportive services, mentors Navigator II staff, and works as part of a multidisciplinary team.
- High school diploma or GED required. Associate degree or higher in a field related to behavioral health preferred.
- Recovery from AOD use and/or GMH for at least one year.
- 90 days experience as a Navigator or Peer Support Specialist.
- 1–3 years full‑time health‑care related experience preferred.
- 1–3 years substance abuse/mental health recovery‑oriented service experience preferred.
- Current valid AZ Driver’s License.
- Clear 39‑month Motor Vehicle Record.
- Behavioral Health Technician (BHT) certification per A.A.C. R9‑10‑115 and CBI Clinical Policies.
- Arizona Fingerprint Clearance card (site specific).
