As a Case Manager on our Integrated Care Team, you'll play a vital role in helping individuals manage chronic medical and psychiatric conditions while supporting their recovery journey.
You'll collaborate closely with members, families, healthcare providers, and community partners to ensure care is coordinated, compassionate, and effective.
Minimum Qualifications:
- High School degree required, and minimum four (4) years of experience in behavioral health and SMI, or
- Bachelor's degree in a related field with one (1) year of related experience.
- AZ valid Driver's License, vehicle with state minimum insurance coverage and able to meet Copa Health company driving requirements and safety sensitive requirements.
- Safety sensitive is any job that includes tasks or duties that the employer in good faith believes could affect the safety or health of the employee performing the task or others
- Bilingual (English/Spanish) preferred.
- Collaborate with the Integrated Care Team to engage, educate, and support service participants
- Develop, manage, and maintain Integrated Service Plans (ISPs) within required timelines
- Participate in comprehensive assessments and ongoing care planning
- Coordinate care with behavioral health, medical, dental, and community providers
- Monitor progress and document outcomes related to recovery and wellness goals
- Ensure services align with ISP goals and regulatory requirements
- Support compliance and program needs through additional duties as assigned
