Position Summary

This position provides clinical assessment, therapeutic interventions, care coordination, transition planning and emotional support to patients and families at Phoenix Children's within their scope of practice. The role participates as an interdisciplinary team member, providing interventions that range from bio-psychosocial assessment, diagnosis, treatment planning, therapeutic interventions, crisis interventions, coordination of care, bereavement support, and transition/discharge planning. The position requires knowledge of family systems, interpersonal dynamics, and the ability to use that knowledge to assist patients and families in achieving therapeutic goals.

Responsibilities also include knowledge of mandated reporter responsibilities, use of financial, community, and other resource networks, coordination with DCS, police jurisdictions, and other governmental authorities, advocacy with outside systems, and collaboration with medical staff. The role may provide individual, group, family therapeutic interventions for an assigned caseload. It is responsible for implementation of patient treatment assignments, management of open patient records, and collaboration with outside referral sources as applicable.

Position

Duties
  • Psychosocial assessments and interventions: assess bio-psychosocial status, mental health status, diagnostic impressions, support systems, safety issues, risk for abuse/neglect, suicide risk assessment, client placement appropriateness, coping skills, family systems, cognitive abilities, etc.; identify therapeutic goals to improve family functioning, mental health, goal attainment, self-sufficiency, medical adherence.
  • Identify barriers to treatment and reconstruction of strengths to aid discharge success; collaborate with interdisciplinary team to set treatment plan goals and objectives and facilitate timely discharges and proactive transition/discharge planning.
  • Documentation: complete clinical record documentation timely; provide info to coordinate care, confirm demographics, recommend disposition planning; complete handoff documentation to Social Work Department per procedures.
  • Coordination of care: develop plans, coordinate effective treatment interventions, devise comprehensive treatment plans, implement therapeutic interventions; maintain communication with medical staff, nursing staff, case managers, peers, families; develop treatment goals and objectives across disciplines; initiate/attend care conferences, interdisciplinary rounds, critical incidents; monitor transitions between outpatient and inpatient services; refer cases to appropriate units; participate in hospital operations initiatives to improve care delivery;

    conduct or facilitate individual, group, family therapy meetings; provide patient/family education; review records for compliance; develop home and level contracts.
  • Education:

    maintain professional competency through educational programs, publications, community committees, professional societies; keep current knowledge of community resources; support patient/family resource connections; receive ongoing education in transplantation; participate in clinical supervision per board requirements.
