Summary

Pediatric Associates is a patient-focused practice whose primary goal is to ensure high-quality healthcare. The primary functions of the Care Coordinator are to ensure timely patient follow‑up and coordinate transitions of care from hospitals and health care systems to the primary care provider while also providing patient education to increase better health outcomes. Care Coordinators help to ensure patients understand every aspect of their care.

Essential Duties and Responsibilities
  • Contacts patients to schedule appropriate visits to meet specific patient needs and increase utilization of preventative care.
  • Reaches out to patients needing preventative screenings and successfully schedules appointments to close gaps in patients’ medical care.
  • Cultivates and supports primary care and specialty provider co‑management with timely communication, inquiry, follow‑up, and information integration into the care plan regarding transitions‑in‑care and referrals. Coordinates the retrieval and upload of discharge summaries, consult notes, and additional information pertinent to patient discharges to the patient’s record.
  • Performs warm calls to newborn patients, welcoming them to the practice, introducing them to our services, and scheduling follow‑up appointments.
  • Registers new patients by gathering pertinent information and entering it into multiple electronic health records, and verifying patient insurance coverage is current. Reconciling and reviewing patient charts to ensure they are accurate during the visit.
  • Promotes adherence to care plans developed by the primary care provider with the patient and family/caregiver(s) and helps increase the patient’s ability for self‑management and shared decision‑making.
  • Engages in our Predictive Modeling campaigns that analyze historical and current patient data, which helps predict future outcomes. Employees will complete outreach on this population to reduce negative patient outcomes.
  • Connects patients with relevant community resources to enhance patient health and well‑being and increase patient satisfaction.
  • Works with patients to plan and monitor care by creating ongoing processes for patients and family/caregiver(s) to determine and request the level of care coordination support needed at any given point in time.
  • Provides patient education on the importance of self‑management and adherence to treatment plans as dictated by the primary care provider. Make adequate contact with families focusing on behavioral goal setting and patient‑centered support planning.
  • Reaches out to our chronic patient population to ensure medication adherence, thereby ensuring the extent to which a patient’s behavior corresponds with the prescribed medication dosing regime, including time, dosing, and interval of medication intake.
  • Protects patient information under HIPAA (Healthcare Insurance Portability and Accountability) laws.
  • Serves as the contact point, advocate, and informational resource for patients, care team, family/caregiver(s), payers, and community resources.
  • Performs other duties as assigned by the Supervisor.
  • Qualifications
    • Education: High School Diploma or equivalent.
      Preferred: Bachelor's degree.
    • Experience: Minimum of 1‑3 years of previous healthcare experience required.
    • Licensure / Certification: Medical Assistant diploma/certificate or Foreign Medical Graduate.
    • Knowledge, Skills, and Abilities:
      • Patient‑facing, but 100% remote
      • Possess effective communication skills and phone presentation in interactions with patients and staff.
      • Ability to utilize appropriate customer service skills in all encounters with patients, parents and/or guardians, professional contacts, and employees.
      • Ability to make a large volume of quality outbound calls a day.
      • Proficient with computers and computer applications (Electronic Health Records, spreadsheets, word documents, electronic mail, and additional systems as needed).
      • Ability to prioritize and organize workload, work with minimal supervision, organize and complete tasks accurately and promptly.
      • Demonstrate skills in effective written and verbal communication.
