**** Deliver physical therapy treatments per PT and physician’s plan of care

* *** Licensed Physical Therapy Assistant (PTA) in the State of Arizona
* Graduate of a CAPTE-approved PTA program (passed national exam)
* Current CPR certification
* Valid driver’s license, insurance, and reliable transportation
