Job Details
- Title: Health and Housing Specialist
- Location: Desert Cove – Phoenix, AZ 85029
- Position Type: Full Time
- Shift: Day Shift (Monday – Friday, 7:30am – 4:30pm or 8:30am – 5:30pm)
- Travel: Up to 80% in the field
- Category: Behavioral Health / Social Work
- Education: High School Diploma/GED required; associate or bachelor in a related field preferred
- Salary: $17.87 – $21.37 per hour, plus $3 per hour differential pay; language differential $1 per hour for Spanish, Vietnamese, Korean, Arabic, Farsi, Swahili, or ASL

- Provide direct, face‑to‑face community‑based services to assist clients in obtaining and maintaining housing.
- Conduct screenings, intakes, and assessments to evaluate presenting problems and develop treatment plan goals.
- Provide outreach to engage clients in PSH services and orient potential clients to the H20 program.
- Arrange transportation for clients as medically necessary, using personal or Terros Health–provided vehicles when available.
- Ensure ongoing collaboration by communicating clinical information with other involved parties.
- Coordinate care with clients’ families, behavioral and general medical providers, community resources, and other supports.
- Assist clients in connecting to primary health services, including follow‑up on medical appointments.
- Deliver excellent customer service to clients, internal and external agencies, and other stakeholders.
- Maintain active caseload of up to 15 clients, contacting each at least four times per week.
- High School Diploma/GED with a minimum of 1.5 years behavioral‑health experience, or associate or bachelor in a behavioral‑health or related field with appropriate experience.
- Interpersonal skills for professional relationships with co‑workers, clients, families, and community members.
- Effective communication skills: non‑judgemental approach, active listening, and positive interactions.
- Ability to work independently, flexibly, and within prescribed deadlines.
- Basic understanding of DSM‑5 (preferred).
- Computer skills:
Microsoft Office suite, electronic medical records;
Next Gen experience preferred. - Valid Arizona driver’s license; legally 21 years of age with at least 3 years of driving experience; meet Terros Health’s driving policy.
- Must pass a drug screen, TB test, and criminal background check.
- Must obtain an Arizona Fingerprint Clearance card (Level1) within 7 working days of assuming the role.
