Client Care Coordinator (Night Shift

Job Summary

Seasonal role, active during peak heat periods from May to October, with potential for continued employment if suitable roles become available. The Client Care Coordinator is a shelter or temporary lodging site-based position responsible for managing a housing caseload, coordinating care for individuals and families experiencing homelessness, and ensuring coordination of care with existing PCMH, PCP or new PCMH, PCP services.

The Coordinator provides coordination and support services to assist individuals or families with ending their current episode of homelessness, monitoring community-based services, facilitating delivery of services, and utilizing housing search software systems to help match individuals to units. The role requires completing data entry in established data systems, providing reports, and documenting coordination work in end-of-shift reports. The Coordinator works as part of a multi-disciplinary team and serves as a nexus, coordinating care with community providers while tracking required interactions per Arizona regulations and program policies.

The Coordinator works on the front lines of delivering lifesaving support to vulnerable populations affected by extreme heat, collaborating with community members, local service providers, and internal teams to distribute resources, share information, and connect people in need with appropriate services. This position requires a strong commitment to community engagement, empathy, and responsiveness to urgent needs of individuals experiencing or at risk of heat-related health issues.

• High school diploma or GED required;
  Associate degree (or higher) in a field related to Behavioral Health preferred.
• Minimum of 12 months of recovery from substance use and/or mental health disorders.
• Minimum of six (6) months of paid related work experience providing exposure to both behavioral and medical crisis situations.
• 1-3 years of full‑time health‑care related work experience preferred.
• Valid identification required.
• Will complete CBI Peer Certification within 90 days of being hired.
• Arizona Fingerprint Clearance Card (Program Specific).
• Strong communication skills, ability to work collaboratively, and competency in data entry.
