Valley wise Health is seeking a Diabetes Educator to serve as a liaison between physicians, patients and education services, promoting seamless diabetes education from hospital to outpatient care. The educator works to integrate patient education across the system and meet regulatory standards.
Qualifications include a Bachelor’s degree in Nursing or Nutrition, at least 3 years in nursing or dietetics, and licensure (AZ RN or RD) with CDCES certification within 18 months of hire.
