Speech Therapist (ST

We are seeking a dedicated and compassionate Speech Therapist (ST) to join our rehabilitation team. The SLP will evaluate, diagnose, and provide therapy for residents with speech, language, cognitive-communication, and swallowing disorders. This role supports residents in maximizing independence, improving communication, and enhancing overall quality of life in a skilled nursing setting.

• Evaluate residents to assess speech, language, voice, cognitive, and swallowing impairments.

• Develop individualized treatment plans based on resident needs and physician orders.

• Provide direct therapy to residents in one-on-one or group settings.

• Educate residents, families, and caregivers on therapy goals, progress, and home carryover techniques.

• Collaborate with interdisciplinary team members (nursing, physical therapy, occupational therapy, social services, dietary) to ensure comprehensive resident care.

• Document evaluations, progress notes, and discharge summaries in compliance with state, federal, and facility guidelines.

• Assist in maintaining compliance with Medicare, Medicaid, and insurance documentation requirements.

• Participate in care conferences and provide input for resident care plans.

• Maintain professional licensure and continuing education requirements.

• Promote a positive, supportive environment for residents and staff.

• Master’s degree in Speech-Language Pathology from an accredited program.

• Current license as a Speech-Language Pathologist in the State of Arizona.

• Certificate of Clinical Competence (CCC-SLP) from ASHA preferred; CFY candidates may be considered.

• Experience in a skilled nursing facility or geriatric setting preferred but not required.

• Strong communication, interpersonal, and organizational skills.
