Home Health Aide

Center Well Senior Primary Care -  - Responsibilities:
Provide direct patient care to patient under the direction of the RN and according to the Aide Plan of Care;
Assist the patient with self-administered medications by opening bottle caps, reading medication labels, and observing medication intake;
Take accurate temperature, pulse and blood pressure measurements and report abnormal results;
Help patient with personal hygiene including bathing, grooming, skin care, dental care, and nail/foot care;
Assist with feeding, mobility, transfers, and ambulation per Plan of Care; document care rendered and communicate with caregivers/doctors/staff
