Physical Therapist – Skilled Nursing & Rehab
Provide evidence-based care to residents in long-term care facilities, developing individualized treatment plans and collaborating with interdisciplinary teams to improve patient outcomes. Travel assignment of 13 weeks in Phoenix, Arizona, offering competitive compensation and comprehensive benefits.
Key Responsibilities:- Evaluate and develop individualized treatment plans for patients in long-term care
- Provide direct patient care, including therapeutic exercises, manual therapy, and functional training
- Monitor patient progress and adjust treatment plans to optimize recovery outcomes
- Collaborate with multidisciplinary team members to ensure comprehensive patient care
- Document patient assessments, treatments, and progress notes accurately in the EMR system
- Bachelor's, Master's, or Doctorate Degree from an accredited physical therapy program
- Current Physical Therapist license in Arizona
- Current CPR certification
- At least 1 year of experience in long-term care environment
- Ability to work day shifts
- Strong communication and interpersonal skills
- Ability to adapt to different clinical settings
Skills:
- Six months of relevant experience preferred
