The Little Foundation was established to provide home and community-based services for individuals with developmental disabilities in the greater Phoenix area. Currently, The Little Foundation offers the following Home and Community-Based Services (HCBS) to eligible members through the Arizona Department of Economic Security, Division of Developmental Disabilities (DES/DDD):
Respite (RSP):
Temporary care provided to give primary caregivers a break. Attendant Care (ATC):
Assistance with daily living activities to help individuals maintain their independence. Habilitation Hourly (HAH):
Individualized support to develop and enhance skills and abilities. Habilitation
- Group Home (HAB):
Structured support in a group home setting to promote skill development and independence. Room and Board
- Group Home (RRB):
Provision of living accommodation and support in a group home setting.

Description
- Direct Care Staff Job Description and Expectations The Direct Care Staff (DCS)'s role is to ensure that the member schedules are followed and that documentation is complete and accurate. The DCS is the frontline worker of the group home and completes the required tasks to ensure we meet the wants/needs of the members. The DCS's role is critical to The Little Foundation's goal of Making the Impossible Possible.

Roles and Responsibilities:

The Direct Care Staff position is paid an hourly wage dependent on experience and knowledge. Direct Care Staff Responsibilities:
Ensuring staffing needs are maintained according to the Planning Document Ensuring staffing ratios are being met, and DCS may be required to stay late if the next staff member is late for a shift Ensuring required documentation is captured accurately and in a timely manner. Transporting members to appointments or scheduled/unscheduled activities Completing/submitting Incident Reports (IRs) timely and accurately to your supervisor Attending PCSP meetings if requested Participating in scheduled activities Working with members to increase independence Completing required training promptly Assisting members with successfully meeting their Planning Outcomes Assuring the Health & Safety of the members Immediately reporting issues to your manager Communicating all staffing concerns and member complaints to the manager Must be on time for scheduled shifts Must have a valid Driver's License (exceptions apply)

DCS Benefits: 16 hours of paid Vacation annually (when requested 2 weeks in advance) 40 hours of paid Sick Time annually (accrued) Annual Evaluations and Pay Raises, dependent on performance 401K Bonus Opportunities An opportunity to grow within the agency Training Opportunity to enhance the lives of the people we serve Pay Rate is $14.70-$17 (DOE) This list is not all-inclusive, as you may be assigned additional tasks as needed/required.

Position Requirements
- Must be able to qualify for a Level 1 Fingerprint Card. Must be available for training within the first 14 days of employment- all trainings are in-house and offered at no cost (must maintain 6 months of employment to receive a copy of certificates.)
