- Develop and implement individualized treatment plans based on patient needs and physician guidance.
- Assess patient conditions, evaluate progress, and modify care plans to meet therapy goals.
- Collaborate with interdisciplinary team members to assess and plan patient care.
- Set and track realistic, achievable goals for residents’ functional improvement.
- Document patient assessments, treatment progress, and outcomes in compliance with regulations.
- Educate patients and families on therapy methods and ongoing care strategies.
- Oversee and participate in screening residents to optimize functional abilities.
- Communicate patient status, needs, and progress effectively with families and healthcare team members.
- Maintain punctuality and consistency to ensure quality patient care.
- Education: Bachelor’s, Master’s, or Doctorate Degree in Physical Therapy from an accredited program.
- Licensure: Current Physical Therapist license in the state of employment.
- Certifications: CPR certification required.
- Experience: Minimum of six months in a long-term care environment preferred.
