Seeking a Discharge Planner to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell clinic.

Full-Time Employed: 40 hours/week - M-F

The Terros Health Discharge Planners serve as a liaison between the inpatient setting and the assigned clinician at the Outpatient site. The Discharge Planner will work with Level I Inpatient Providers to review presenting clinical information.

Responsibilities:

  • Review presenting clinical information, identify resources, and help coordinate successful discharge into the community and follow up with the clinical team post discharge.
  • Activities will be performed both in inpatient and outpatient settings (i.e., inpatient visits, coordination meetings, service planning).
  • Connect individuals receiving Level I inpatient services with ongoing Terros Outpatient treatment upon discharge.
  • The Discharge Planner will work with the clients while in the hospital, hospital unit staff, outpatient team, member family, PCPs, and health plans as clinically indicated to ensure an effective transition.
  • Participation in individualized service planning and execution of written discharge plans.
  • Offer coordination to an array of services that actively address recovery goals.
  • Reduce the risk of readmission to hospital, reduce the utilization of Crisis services and improve the coordination with the PCP and Terros psychiatric provider.
