The Lung Institute at Banner University Medical Center Phoenix provides highly specialized care to patients battling an array of chronic, advanced, and obstructive lung diseases. Pulmonologists, cardiologists, thoracic surgeons, respiratory therapists, pathologists and other health care experts collectively tailor treatment plans that may include medical, surgical and other therapies to treat everything from asthma to lung cancer.
The Patient Care Coordinator will serve as a provider Liaison to all staff and services. The PCC will receive instruction from the provider and ensure its completion by delegating to the appropriate team member. On occasion, the PCC will reach out to the appropriate staff to obtain a status update. The PCC is not a leader role; however, they are responsible for establishing effective communication within their assigned service line for the completion of all tasks and requests.
Along with being a Liaison, the PCC role is responsible for reviewing completed DRs and updating status via ARM tool, scheduling new and established visits, completing outgoing referrals for their service line, monitoring messages via PCC pool and maintaining prompt and effective communication with patients, providers, offices, vendors and PCC for the PM&R service line to support physicians with procedure scheduling, authorization, and coordination.

This position is responsible for providing personalized coordination, clarification and communication of all administrative aspects of care including patient needs assessments, insurance and authorization verification, registration, maintaining and handling of documentation, and scheduling of appointments. This position partners with the clinical care team to ensure a seamless experience for the patient and their family across the entire continuum of their treatment.
This position assists with providing resources to help the patient maintain optimal care. This position performs follow‑up tasks identified during the patient needs assessment for management of patients across the healthcare continuum or when the patient is in the continuum and needs additional resource support.
