Accent Care is seeking a compassionate hospice social worker to join our pre-hospice/hospice team, coordinating social, spiritual, and emotional aspects of patient care and supporting families through challenging transitions.
You will collaborate with an interdisciplinary team to develop and review plans of care, provide discharge planning, bereavement support, and participate in ethics consultations and committees. On-call coverage and supervision of interns may be required.
