Caregiver – Home Well Care Services

We are looking for compassionate, dependable, and professional individuals who want to make a meaningful difference in the lives of seniors and individuals who need support to remain safe and independent a Home Well caregiver, you'll provide personalized, one-on-one care based on each client's individualized care plan while building meaningful relationships along the way.
Depending on the client's needs and care plan, responsibilities may include:
- Companionship and emotional support
- Meal preparation and assistance with meals
- Light housekeeping and laundry
- Shopping and errands
- Transportation to appointments and outings
- Medication reminders
- Personal care, including bathing, grooming, dressing, and toileting
- Assistance with walking, mobility, and transfers
- Supporting activities of daily living
- Observing and reporting changes in a client's condition
- Following individualized care plans and company policies
What we're looking for we are seeking caregivers who are:
- Caring, compassionate, patient, and respectful
- Mature, professional, reliable, and punctual
- Dependable and committed to their clients
- Strong communicators
- Comfortable working independently
- Able to follow care plans and instructions
- Physically able to safely perform required duties
Qualifications & requirements
- 1,000 hours of caregiving experience preferred, including approximately 6 months full-time or 12 months part-time experience with seniors or individuals with chronic illnesses
- CNA or HHA certification preferred, but not required
- Exceptions may be considered for newly licensed CNAs/HHAs and companionship or housekeeping-only caregivers
- Current TB test
- Reliable transportation
- Valid driver's license and current auto insurance if transporting clients
- Ability to safely assist with client transfers
- Ability to pass a criminal background check
- Verifiable professional references
Essential responsibilities
Follow care plans:Provide services according to each client's individualized care plan and communicate changes to the office.
Quality care:Provide compassionate, respectful, and dependable care while promoting client dignity and independence.
Communication:Promptly report changes in condition, concerns, incidents, or emergencies to the Care Manager.
Safety:Follow infection control practices, safety procedures, and PPE requirements when applicable.
Timekeeping:Accurately clock in and out of every shift and document completed tasks using Clear Care Telephony.
Professionalism:Maintain a professional appearance, follow the company dress code, and represent Home Well positively.
Reliability:Arrive on time, attend required training and meetings, and follow company policies and procedures. Physical & environmental requirements
- Safely assist clients with transfers and mobility
- Perform essential caregiving duties in client homes
- Travel to client homes using reliable transportation
- Work in a variety of home environments

Compensation: $16.00 - $20.00 per hour
