Admissions Liaison

Encompass Health seeks an Admissions Liaison to support patient access to our inpatient rehabilitation services. In this role, you will respond to referrals, complete pre-admission assessments, verify clinical and financial eligibility, and educate patients, families, and referral sources on our programs. You will coordinate with physicians, case managers, and the rehab team to ensure safe, timely transitions, while building strong relationships with hospitals and community partners.

Responsibilities

• Respond to and manage inpatient rehabilitation referrals from hospitals and community sources
• Conduct timely pre-admission clinical assessments to determine rehab appropriateness
• Verify insurance coverage and assist with financial and authorization processes
• Educate patients, families, and referral sources on rehab programs, services, and expectations
• Coordinate admission logistics to ensure safe, timely transitions into the rehab hospital
• Collaborate with physicians, case managers, and interdisciplinary teams on admission decisions
• Maintain accurate documentation in EMR and tracking systems for referrals and outcomes
• Develop and maintain relationships with referral sources to support census growth
• Participate in community outreach, presentations, and marketing activities as needed
• Monitor admission metrics and contribute to process improvement initiatives
