Direct Care Worker

Mosaic Life Care seeks a compassionate Direct Care Worker to support patients with daily living activities and promote independence in a patient-centered environment. In this role, you'll provide hands-on personal care, assist with mobility, hygiene, meals, and safety, and observe and report changes in condition to the clinical team. You'll work closely with nurses and other caregivers to follow individualized care plans.

Responsibilities

• Provide compassionate direct care and daily living assistance to patients in home and facility settings
• Support patients with bathing, grooming, toileting, dressing, and mobility
• Observe and report changes in patient condition to clinical staff promptly
• Assist with meals, hydration, and safe feeding as needed
• Promote a safe, clean, and dignified environment for all patients
• Accurately document care and follow care plans and safety protocols
• Collaborate with nurses and the care team to support patient goals
