The Case Manager works under the direction of the Executive Director. The Case Manager assists during the admission and discharge process of residents and coordinates meetings with residents during their stay to evaluate and effectively contribute to an individualized plan of care and discharge goals. The Case Manager works closely with fellow IDT members to manage skilled services and ensure documentation is in place to support skilled coverage.

• Background Screening, Licensure Verification (if applicable), Reference Check, Drug Testing
• Tuberculosis Testing
• Employee Health Screening (Conducted after Hire)
• Ability to Move, Lift and Transfer a minimum of 50 Pounds
• Must be able to Speak, Read, Write and Understand English
