Job Summary:

This position will serve as the department's designated subject matter expert/specialist to inform and educate policy makers, administrators, and the public about the importance of prevention, diagnosis, and management of diabetes and pre-diabetes, and will manage the activities and projects of the Diabetes Prevention and Control Program. This position reflects the duties that are required to interact with chronic disease and diabetes community partners, stakeholders, voluntaries (i.

e. American Diabetes Association, Arizona Diabetes Coalition and Arizona Diabetes Leadership Council) and outside agencies; in addition to bureau, division and senior management within ADHS. This position is also required to maintain a direct confidential working relationship with the chronic disease office chief and bureau chief. This Health Program Manager III position is responsible for fostering collaboration, which will produce innovative and cutting-edge approaches to chronic disease and diabetes prevention and control efforts throughout the state.

The position will be responsible for addressing the common communication elements of chronic disease, diabetes and primary risk factor categories from a comprehensive and integrated perspective. This position will be enforcing policies and procedures within the bureau as well as the department. This position works with county & community partners; in addition to stake holders on chronic disease and diabetes projects that foster collaboration with health authorities, voluntaries and outside agencies.

Duties include:

Developing, implementing, monitoring and reporting activities of the Diabetes Prevention and Control Program, and the Office of Chronic Disease. Provides the direction of program and resolutions to program problems. Develops and monitors an accurate budget request that corresponds with the program's assigned grant(s) and monitors expenditures. Develops and monitors program contracts and required contractual processes. Prepares and submits in a timely manner all program reports and grant applications.

Develops and maintains relationships with internal and external partners. Interprets state and federal laws and local rules and regulations governing aspects of all implemented program elements. Is responsible for work of considerable difficulty in planning, implementing and administering a major statewide health program.

• Manages the Diabetes Prevention and Control Program in accordance with the funding source guidance from the Centers for Disease Control and Prevention to increase the availability and continuity of services for diabetes and pre-diabetes.
• Serves as the department's designated subject matter expert/specialist to inform and educate state policy makers, program administrators, and the public about the importance of prevention, diagnosis, and management of diabetes and pre-diabetes, and represents ADHS nationally in matters related to chronic disease, diabetes and pre-diabetes.
• Prepares and manages contracts with service providers to carry out the program’s scope of work.
• Develops and implements program evaluations, needs assessments, surveys, logic models, as necessary for program development and planning.
• Develops and maintains relationships with internal and external partners (American Diabetes Association, Arizona Diabetes Coalition, AHCCCS, Universities and non‑profits and local/state coalitions) to maximize the effectiveness of the program.
