Alignment Health is seeking a compassionate, organized, and motivated Bilingual Spanish Health Coach & Educator to support members throughout Maricopa County, Arizona.
This is a highly field-based position focused on conducting in-home visits with members across the community. The Health Coach & Educator builds trusted relationships with members, caregivers, and families by providing wellness coaching, health education, support, and encouragement to help improve overall health and well-being. Success in this role requires comfort working independently in the field, managing a daily schedule of member appointments, and traveling throughout an assigned service area.

- Conduct approximately 6-8 in-home member visits per day
- Spend the majority of your workday in the field meeting members in their homes
- Build trusted relationships with members, caregivers, and families through health coaching and education
- Work independently while managing a daily schedule of member visits
- Daily local travel is an essential component of this role
- Member visits are generally organized by geographic service areas or quadrants to support efficient scheduling and reduce driving time
- Mileage reimbursement is provided from the time you leave home until you return home
