Circle the City seeks a Health Navigator to work with indigent, unhoused, and uninsured patients at partner Emergency Departments in Phoenix. You will screen, refer, and assist patients to access housing, benefits, and a medical home, while coordinating with community partners for shelter and services.
Join a multidisciplinary team to reduce unnecessary ER use, document interactions, and advocate for patients through referrals to Circle the City's Medical Respite Centers and other supports.
