Our mission-driven healthcare organization in the Phoenix area is dedicated to improving health outcomes across diverse communities. In this Monday–Friday role, you will provide evidence-based Medical Nutrition Therapy in community and outpatient settings, conducting nutritional assessments, developing individualized care plans, and delivering culturally and linguistically appropriate counseling alongside a supportive interdisciplinary team.

  Monday–Friday, 8:00 AM–5:00 PM
• Competitive salary: $62,000–70,000 annually (DOE)

• Conduct comprehensive nutritional assessments, including medical history, dietary intake, anthropometrics, and relevant lab data
• Develop, implement, and monitor individualized Medical Nutrition Therapy care plans for patients with chronic and acute conditions
• Provide one-on-one and group nutrition counseling using motivational interviewing techniques
• Train, mentor, and evaluate local agency WIC staff, providing on-site and telephone technical assistance
• Oversee and approve food and formula special authorizations, coordinating with medical providers as needed
• Deliver culturally and linguistically appropriate nutrition education to diverse populations
• Document patient encounters, assessments, and outcomes accurately in the EHR
• Travel bi-monthly to clinic and community sites in the Phoenix area to provide on-site nutrition services

• Credentialed as a Registered Dietitian (RD) by the Commission on Dietetic Registration (CDR)
• Minimum 1 year of experience as an RD
• Valid driver's license and clean 5-year driving record required
• Must be able to pass a background check
• Knowledge of the WIC program
• Bilingual in Spanish preferred
