Summary

The Behavioral Health Professional provides individual, group, and family therapy, psychosocial assessments, crisis intervention, formulates treatment plans and recommendations, discharge planning, care coordination, therapeutic interventions, and consultation services for behavioral health patients and families. Assists the interdisciplinary treatment team in addressing emotional, mental health, substance use, psychosocial, and environmental factors impacting patient care and recovery. Facilitate referrals and transitions to appropriate levels of care and community resources.

Responsibilities performed will align with the clinician’s licensure, scope of practice, competency validation, and hospital policy.

Responsibilities
  • Facilitates day, afternoon, or evening Intensive Outpatient Program (IOP) therapy groups, including psycho educational, therapeutic, process, and recovery-focused groups in accordance with licensure, competency validation, department needs, and hospital policy.
  • Conducts psychosocial assessments, crisis evaluations, and behavioral health screenings to identify patient needs, barriers to treatment, safety concerns, and discharge planning requirements while completing documentation within established departmental timelines.
  • Collaborates with physicians, nursing staff, intake personnel, emergency department staff, case management, referral sources, and interdisciplinary team members to support treatment planning, patient throughput, continuity of care, and timely behavioral health evaluations.
  • Facilitates discharge planning activities, including coordination of aftercare services, outpatient appointments, behavioral health referrals, community resources, housing assistance, transportation needs, medication access, and continuity of care services.
  • Conducts individual, family, psycho educational, therapeutic, and process groups in accordance with licensure, competency validation, department needs, and hospital policy.
  • ADD:
    Develop and update individualized treatment plans.
  • Documents assessments, interventions, treatment recommendations, discharge planning activities, patient progress, and care coordination activities in accordance with hospital policy.
  • Maintains knowledge of behavioral health practices, crisis management, trauma-informed care, therapeutic communication, safety protocols, and electronic medical record documentation while participating in departmental quality improvement initiatives and performance improvement activities.
  • Provides services to adult, geriatric, substance use, and/or co-occurring disorder populations depending on department assignment and operational needs.

Minimum Requirements
  • Master’s Degree in Social Work, Counseling, Marriage and Family Therapy, or related behavioral health field required.
  • Licensure/Certification:

    Current Arizona licensure or license eligibility in one of the following: LMSW (License Master Social Worker), LCSM (License Counseling Social Worker), LPC (License Practical Counselor)
