Circle of Life Hospice & Palliative Care is seeking a compassionate and dedicated PRN Hospice Chaplain to provide spiritual and emotional support to hospice patients and their families.

The Chaplain will work as part of the interdisciplinary hospice team, providing spiritual care that respects each patient's beliefs, values, culture, and personal wishes.

Responsibilities
  • Provide spiritual and emotional support to hospice patients and their families.
  • Conduct spiritual assessments and identify patient and family spiritual needs.
  • Visit patients in their homes, facilities, and other care settings as needed.
  • Provide prayer, counseling, spiritual guidance, scripture reading, or other appropriate spiritual services based on the patient's wishes.
  • Respect and support patients of all faiths, beliefs, and spiritual backgrounds.
  • Provide support during times of crisis, grief, and end-of-life transition.
  • Assist patients and families with advance care planning and end-of-life concerns when appropriate.
  • Provide bereavement and grief support to patients and families as appropriate.
  • Participate in interdisciplinary team meetings and communicate patient and family needs to the hospice team.
  • Maintain timely and accurate documentation of visits and services provided.
  • Collaborate with nurses, social workers, aides, physicians, and other members of the hospice team.
  • Maintain confidentiality and comply with HIPAA and hospice regulations.
  • Participate in hospice quality improvement and required meetings or training.
Qualifications
  • Ordination, commissioning, or appropriate authorization from a recognized faith organization preferred.
  • Clinical Pastoral Education (CPE) or hospice chaplain experience preferred.
  • Previous experience providing spiritual care in a healthcare, hospice, hospital, or community setting preferred.
  • Strong communication and interpersonal skills.
  • Compassionate, respectful, and comfortable working with patients and families from diverse backgrounds.
  • Ability to work independently and manage a flexible PRN schedule.
  • Reliable transportation and a valid driver's license.
  • Ability to travel to patient homes and facilities throughout the service area.
