Seeking a Rehabilitation Specialist to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Mitchell Recovery Health Center.

Responsibilities:
- Work collaboratively with the clinical team to engage, educate, communicate, and coordinate care with patients, their families, behavioral health, medical and dental providers, community resources, and others in ensuring that all services prescribed in the individualized service plan are implemented.
- Conduct face-to-face interactions with patients, family, or other involved parties in the home, community, and recovery clinic for the purpose of assessing, maintaining, or enhancing a person's functioning.
- Participate in Consumer Advisory Council at the respective site. Promote patient and family/natural support participation in Advisory Council meetings.
- Facilitate peer support and psychosocial groups, either in a clinic or community setting, and engage patients in group participation as related to their individual recovery goals.
- Provide transportation for the patients as appropriate and as medically necessary, and determined by the clinical team, in a personal vehicle or in Terros Health provided vehicle, when available. Coordinate alternative transportation for patients and identified natural supports, as medically necessary.
- Provide excellent customer service to patients, internal and external provider agencies, state entities, or other stakeholders (e.g., probation officers, advocates, family members, hospital staff, jail staff, vocational rehabilitation, etc.) as needed.
- Provide direct assistance in finding necessary resources other than covered services to meet basic needs for patients served.
- Encourage, assist, and support patients in expressing their perspective throughout the processes of assessment, service planning, and service delivery, while maintaining a focus on their recovery.
