Earn More with Shift Differentials
- +$2/hr – Monday-Thursday Nights
- +$3/hr – Friday-Sunday Nights
- +$2/hr – Saturday-Sunday Days
- Manage care of individuals in crisis, stabilizing them from acute episodes and facilitating admission intakes, treatment planning, discharge planning, collateral gathering, legal assistance, and initiation of personalized care plans.
- Work closely with an Interdisciplinary Treatment Team to coordinate care and discharge planning with families, local providers, and community partners, carrying a caseload and acting as an ambassador to community partners.
- Collaborate with the Crisis Therapist to develop and implement individualized person‑centered Treatment Plans appropriate for brief intervention.
- Gather collateral from the individual and involved parties to provide a thorough assessment of immediate treatment and discharge needs.
- Actively coordinate care to ensure services align with facility staff, identify and address gaps in service needs, and recommend solutions to meet patients’ needs upon discharge.
- Engage, monitor, and communicate with the individual with the Interdisciplinary team to maintain safety while meeting individual needs.
- Maintain clinic records, documenting activities, obtaining necessary signatures, and recording services in the medical record.
- Maintain knowledge of the Title 36 Involuntary Commitment process, monitor petitions for timeliness, ensure forms are complete, and attend COE hearings when subpoenaed.
- Maintain knowledge of the Maricopa County RBHA system and potential funding sources.
- Participate in individual and group supervision, ongoing education, training, and performance assessments as required, and perform additional duties as assigned.
- Associate’s degree in behavioral health or a related field
- At least 2 years of behavioral health experience
- Compliance with the Company’s mandatory vaccination policy (fully vaccinated or exempt)
- At least 4 years of behavioral health experience
- Bachelor’s degree in behavioral health or a related field
- Knowledge and experience with the AZ Title‑36 legal process
