Spectrum Healthcare is looking for a compassionate, organized, and dependable Care Manager to join our team. This position is a great opportunity for someone who wants meaningful work and enjoys making a real difference in the lives of others.

As a Care Manager, you will help clients navigate behavioral health services, healthcare needs, and community resources. You will work closely with clients, families, healthcare providers, community organizations, and members of the treatment team to coordinate services and support each clients treatment goals.

This is a hybrid position that includes work in the community, office, and remote setting. No two days will look exactly the same. One day you may be meeting with a client, and the next you may be coordinating services with a provider or helping someone access an important community resource.

As a Care Manager, you will:

• Build professional and supportive relationships with clients.
• Help clients identify their needs, strengths, goals, and available resources.
• Coordinate behavioral health, physical health, and community based services.
• Connect clients with housing, transportation, healthcare, benefits, education, employment, and other community resources.
• Complete initial assessments and reassessments under appropriate supervision.
• Help develop, implement, and update individualized treatment plans.
• Provide supportive counseling, rehabilitation services, and care management interventions.
• Communicate with healthcare providers, behavioral health professionals, community agencies, and other members of the treatment team.
• Participate in client staffing meetings, case consultations, supervision, and team meetings.
• Complete accurate and timely documentation in the electronic health record.
• Make sure documented services clearly support the clients treatment goals and progress.
• Maintain appropriate professional boundaries during all client interactions.
• Protect confidential client information and follow HIPAA requirements.
• Follow clinical and supervisory direction related to client care and assigned responsibilities.
• Report safety concerns, incidents, and changes in client needs through the proper channels.
• Manage assigned caseload responsibilities and communicate when additional support is needed.
• Provide culturally respectful services to individuals from different backgrounds and life experiences.
• Follow all organizational, legal, regulatory, licensing, and payer requirements.

Your day may include checking in with clients, helping someone schedule a medical appointment, locating housing or transportation resources, communicating with a treatment provider, completing progress notes, attending a client staffing meeting, and working with your supervisor to address changes in a clients needs.

Some days may be busy and require you to balance several priorities ong organization, communication, and follow through will help you succeed in this position.
