HOPE ~ HEALTH ~ HEALING Seeking a Rehabilitation Specialist to join our interdisciplinary team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Oak Recovery Health Center.

Duties Include:

Assist clients with identifying treatment and recovery goals, and provide supportive services to help clients reach their employment, education, and community involvement goals.

Implement services identified in Individual Service Plans, and coordinate rehabilitation-related services for clients served on the clinical team.

Communicate and document client progress toward their recovery.

Embrace the recovery philosophy and promotes a safe recovery environment for clients.

Use brief clinical interventions, Motivational Interviewing, and supportive listening skills to engage and support clients receiving services.

Engage and provide direct support to clients’ natural support systems.

Work in partnership with co-located Employment Specialists and RSA Vocational Rehabilitation Counselors, community program providers, and other parties involved to assist clients in maintaining employment, education, and other meaningful daily activities.
