Under supervision of site management, the Clinical Housing Specialist performs duties in support of therapeutic services and provides comprehensive direct support services which are structured to meet the needs of an adult population composed of SMI, dual diagnosed and/or referred persons. Responsible for working with the member to facilitate access to housing opportunities, address housing permanency and support the independent living in a community setting;

performs related duties as required.

• Assess client needs and provide tailored housing resources.
• Accompanies members on occasion to housing briefings and/or to explore housing options.
• Tracks applications and referrals to housing programs and services for members.
• Maintains accurate and up-to-date data demographics related to housing and homeless status.
• Collaborate with community organizations to connect clients with social services that enhance their stability.
• Utilize motivational interviewing techniques to empower clients in their housing journey.
• Stay informed about changes in housing policies and advocate for client rights within the community.
