Signify Health, part of CVS Health, seeks a Full Time Clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations across a defined coverage area. You will travel up to 100% of the time, conducting assessments with an emphasis on connecting patients to appropriate care in their homes.
Applicants must hold an active license, be board-certified, and be able to obtain multi-state licenses. 40-hour weeks with a comprehensive benefits package are offered.
