Duties
  • Conducts client intakes and maintains the quality, accuracy, and completeness of medical record
  • Facilitates groups as assigned and documents interventions and client responses.
  • Participates in treatment meetings and supports client orientation, treatment planning, and progress review.
  • Engages clients, coordinates referrals and outreach activities, and documents all care coordination efforts.
  • Shares client observations with the clinical team to support treatment planning and ensure client and staff safety.
  • Coordinates and provides client transportation, including route planning, client communication, and collaboration with transportation partners.
  • Reports safety concerns promptly and follows established safety protocols.
  • Maintains community resources and discharge planning information to support continuity of care and community reintegration.
  • Monitors and maintains program vehicles in accordance with Terros Health policies.

Qualifications
  • Education – High School Diploma or GED, as well as a combination of behavioral health education and work experience totaling a minimum of three years or an associate’s degree in any field, plus two years of work experience in a behavioral health service delivery.
