The F/ACT Integrated Care RN is a Registered Nurse with experience working with patients who have a serious mental illness with co‑occurring substance use disorder and/or chronic health conditions. The F/ACT Integrated Care RN provides nursing services to all patients assigned to their respective F/ACT team. The RN is responsible to assist patients with disease management, health and wellness activities and coordination of care.

Services include one‑on‑one psychiatric nursing care, engagement, assessment, treatment planning, medication education, health risk assessment, attending Primary Care Physician (PCP) and specialty appointments with patients, ongoing coordination of care with primary care doctors and specialists, assistance to psychiatric providers, oversight of court‑ordered treatment and completing Court Ordered Treatment (COT) reports.

• Associates Degree from an accredited school of nursing required. Bachelor’s Degree from an accredited school of nursing preferred.
• 1–3 years of Health Care related experience where his or her prior experience would provide adequate exposure to both behavioral and medical situations preferred.
• Additional experience in multiple settings and levels of care, including both mental health and substance abuse preferred.
• Valid Arizona (or Compact) Board of Nursing Registered Nurse licensure (current and in good standing without restrictions). Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI Clinical Policies and Procedures.
• Current AZ Driver’s License (valid and in good standing).
• Clear 39‑month Motor Vehicle Record.
