Are you a compassionate Registered Nurse (RN) who finds purpose in supporting patients and families through meaningful moments of care? Hospice of the West is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end‑of‑life care throughout the community.
ScheduleMonday – Friday, 8 am – 5 pm
TerritoriesWest Valley (Peoria, Sun City, and Glendale)
In this rewarding role, you’ll deliver hands‑on hospice nursing care, coordinate services, and guide patients and families through every step of their hospice journey. As a valued member of our interdisciplinary care team, you’ll help ensure every patient receives the best care possible – care grounded in compassion, clinical excellence, and connection.

- Respond to clinical referrals with care and expertise, addressing inquiries and requests with a compassionate touch.
- Partner closely with the Clinical Director to assess eligibility and suitability of clients for specialized hospice services.
- Provide direct care and support for patients, ensuring comfort and quality of life remain paramount.
- Deliver top‑notch nursing care rooted in proven principles and techniques to ease pain and bring solace.
- Educate and advocate for patients and families, encouraging active participation in personalized care plans.
- Participate in call duties, ensuring presence when needed most.
- Current, unencumbered Registered Nurse license.
- Minimum one year of RN experience in a medical‑surgical/acute‑care setting; hospice experience is a plus.
- Computer proficiency, including EMR use.
- Current CPR certification if required by state.
- Valid driver’s license, vehicle insurance, and reliable transportation.
$83,000 – $94,000 per year (average pay range). Final compensation will be determined based on relevant experience, qualifications, and responsibilities.
