Hospice Admissions RN - West Valley

The registered nurse plans, organizes and directs hospice care in the West Valley of Arizona, ensuring comprehensive and ongoing assessments, care planning, and coordination with interdisciplinary teams. This full‑time role emphasizes community health education and delivery of individualized care to patients and families in homes and communities.

• Patient Care
  • Completes an initial, comprehensive and ongoing assessment of patient and family to determine hospice needs.
  • Provides a complete physical assessment and history of current and previous illnesses.
  • Utilizes all elements of the nursing process to deliver professional nursing care.
  • Writes and initiates a plan of care, regularly re‑evaluating patient and family/caregiver needs and revises the plan as necessary.
  • Uses health assessment data to determine nursing diagnoses and develop care plans that establish goals and incorporate palliative actions.
  • Includes the patient and family in the planning process and initiates appropriate preventive and rehabilitative nursing procedures.
  • Administers medications and treatments as prescribed by the physician’s plan of care.
  • Provides counseling to the patient and family and supplies health care instructions according to assessment and plan.
  • Assists the patient with activities of daily living and facilitates efforts toward self‑sufficiency and optional comfort care.
• Case Management
  • Assumes responsibility to coordinate patient care for the assigned caseload.
• Communication
  • Maintains accurate clinical notes and records pain/symptom management changes and outcomes.
  • Communicates with the physician regarding patient needs, reports changes, and obtains orders.
  • Coordinates care with community health persons and teaches patient/family self‑care techniques.
  • Shares medication, diet, and other instructions; recognizes opportunities for health counseling.
  • Works in concert with the interdisciplinary group; attends meetings and supports continuity of care.
  • Encourages development of relationships with partners and referral sources.
• Professional Development
  • Participates in on‑call duties as defined by policy.
  • Assures arrangements for equipment and services are available.
  • Maintains and upgrades professional knowledge through continuing education and in‑service classes.

• Currently licensed as a Registered Nurse (RN) in the State of Arizona.
• Hospice or home health experience strongly preferred.
• Strong clinical judgment with ability to assess and prioritize patient needs.
• Excellent communication, organization, and problem‑solving skills.
• Comfortable working independently with confidence and professionalism.
• CPR certification required.
• Valid Arizona driver’s license, reliable vehicle, and current auto insurance.
